- CCA Online Request

All online requests are hosted securely by UC Cooperative Extension. You will need to email your meeting program to joyce@capca.com after filling out and submitting the online form.
- CCA Mail-in or Fax Request

To complete mail-in requests, you will need to print, fill out, and mail the respective mail-in form. You may choose to download and edit a word document directly or print a Pdf file and edit it by hand.

To apply for a California PCA/DPR Continuing Education Approval, you need to print, fill-out, and mail the request form AND either include a check, money order, or completed visa payment form. This is because of a nonrefundable $45 course review fee.
